In a symposium conducted in city, renowned joint replacement surgeons from across the country endorsed the highly advanced and effective Patient Specific Jig technology which is now made available in India for benefit of patients suffering from knee joint disorders. The technology was made available to 10 select knee replacement surgeons in North India for an early user feedback. 10 surgeons from northern India attended the symposium to learn about the early user feedback and patient outcomes of this new technology. Surgeons who were involved in designing and developing of this new technology were present on the occasion. Amongst them were Dr. (Prof) Raju Vaishya, Sr. Consultant, Apollo Hospital (Delhi), Dr. Sanjai Srivastava, Sr. Consultant Mayo Hospital, Lucknow, Dr. Vikas Kulshreshtha, Group Captain, R&R Hospital (New Delhi). This is the first customized jig that has been developed and manufactured in India. Dr. (Prof) Raju Vaishya recently conducted a study on usage of customized jig technology on Indian patients at Indraprastha Apollo Hospitals, New Delhi and shared the study outcomes at the forum. As part of the program, over 125 surgeries have been successfully performed. Excellent patient outcomes have been observed with less operating time reduced blood loss, faster recovery and shorter hospital stay. No post op complications have been reported during the feedback program. In the day-long symposium organized on the eve of the National Joint and Bone Day, senior joint replacement surgeons from different corners of county congregated to comprehend the benefits of the new technology and its pressing need in the background of enhancing patient outcomes and surgical success. Speaking with media at a round table discussion, Dr. Sanjai Srivastava explained that Correct Alignment of the artificial knee implant is the key to successful surgery. Closer the implant alignment is to the natural knee, better will be the surgical outcome. Traditionally, achieving this alignment was based on experience, judgment and skill of the surgeon. While experienced surgeons may near-accurate alignment most of the times, but there is a significant dependence on human competence. With the advent of customized jig technology, the accurate alignment for individual patient can be calculated much before the surgery based on his CT scan. The patient’s CT scan and advanced computer analysis help arriving at the most accurate alignment for that particular patient, basis which a precise jig is manufactured. This jig, which is customized for each patient, is used to give the most appropriate bone cuts and positioning of the implant during the surgery. “Apart from the efficient surgical procedure, customized jig also improves knee functions post-surgery and reduces the chances of revision surgery. Since the product is manufactured in India, patient doesn’t have to wait for long” explained Dr. (Prof) Raju Vaishya. “Almost 30% of key surgical steps are eliminated with the help of this new technology which is also less invasive. As a result, the operative is also reduced by one third,” said Dr. Srivastava. Talking about their recent publications in prestigious journals(International Orthopaedics and Journal of Arthroplasty) on their experience on customised jig technology on Indian patients, Dr.(Prof) Raju Vaishya said that we are convinced that customized cutting blocks may offer certain otheradvantages like decreased operative time, lesser bleeding, and faster recovery. The major disadvantages of this technology until now used to be an increased cost and delay in their procurement for the surgery. However, this has now been overcome by the local manufacturing of these blocks by an MNC and this lag time has been reducedto five to seven working days only and the cost for a single kneeblock has also come down by 1/3rd. We alsobelieve that the additional cost of customized jigs can beoffset by decreased operative time, less need for bloodtransfusion and early discharge from the hospital.

It’s not just that with rising elderly population,Parkinson’s disease is expected to rise; experts warn that it might impinge on men more strongly as compared to women
Settled career, doting wife and loving daughter; Mr. Sidhaarth Sinha, a 43-year-old football coach, has been living his life the way he always dreamt of, but only until he was diagnosed with Parkinson’s disease recently.
It all started a couple of years back with tremors in his hands, which soon followed slurred speech and inflexibility in his limbs, leaving him deeply perturbed and almost entirely uncertain about his professional as well as personal life.
Mr. Sinha’s is not an exceptional case, experts implicate that Parkinson’s disease is fast becoming the next male disease. According to Journal of Neurology Neurosurgery and Psychiatry,men are 1.5 times more likely to develop Parkinson’s disease as compared to women. Explains Dr Vinay Goyal, Professor, Department of Neurology,
All India Institute of Medical Sciences, “Of late, the incidence of Parkinson’s disease is not just increasing among younger people, it is also affecting men more as compared to women.Even though we need to conduct more research on this to come up with a rational explanation, some studies indicate that probably oestrogen hormone is protecting women from degenerative disease.”
There are certain diseases like coronary heart disease or osteoporosis which have been found to be unevenly prevalent among the sexes and are addressed accordingly. Experts suggest that scientific evidences and difference in male and female physiology have been a good rationale behind this gender specific prevalence of diseases. However, recently few research studies are implicating that Parkinson’s might as well make an entry into this category, which apparently has no such clear explanations.
Bread earning role played by men traditionally makes it important that situation needs to be addressed effectively and quickly. Fortunately significant improvement in treatment modalities in recent past have come up as effective ways of managing symptoms of Parkinson’s disease; making them employed them effectively.
As in the case of Mr. Sinha,who,despite being almost completely shattered by the chain of events, could not afford to give up at so young an age, especially when he has a family to support. Since his doctor had already made it clear that Parkinson’s is a degenerative disease and cannot be cured, and considering his job requirements, he consciously shifted his focus on symptomatic relief, and it was then that he got to know about Deep Brain Stimulation (DBS) technology as an excellent option.
“DBS is a therapy in which the specific cells in brain are stimulated by putting in a lead. The leads are connected to a pacemaker that delivers continuous stimulation to the brain cells. Stimulation of these areas enables the brain circuits that control body movements to function better. Recent studies have suggested that advising this surgery might be a good idea to have best outcome of the therapy,” says Dr Dr Vinay Goyal.
Experts state that patients not capable of controlling their movements despite various combinations of medications and requiring significant relief due to their work requirement, or if they experience serious side effects of the medication, DBS Therapy is a good option for them.
Parkinson’s disease is a progressive disorder of nervous system, which primarily affects movements and speech, usually staring with tremors, stiffness and slowing of movement.
Conventionally, medication in Parkinson’s disease is used to improve motor functions; however considering degenerative nature of Parkinson’s disease it is likely that they lose their effectiveness over time, cause side effects, or both. Moreover, as the condition progresses, the medication levels required for motor function control may cause intolerable or undesirable side effects.
After undergoing the DBS therapy, Mr. Sinha’s life almost immediately started coming back to normalcy. The treatment not just helped managing his condition and improving his quality of life but more importantly made him believe that he can still carry on his responsibilities as male member of the family, that toowhile pursuing a challenging profession of football coach.
Symptoms of Parkinson's disease alongsidedeteriorating overall quality of life could make even day to day activities an ordeal to carry, if not addressed effectively. While there are less people like Mr. Sinha, who preferredDBS, offering an adjustable and reversible treatment, there are many others who remain either oblivious or apprehensive toward such advanced treatment options.
So, with evidences indicating the rise in Parkinson’s disease among men, alongside identifying the reasons behind it and handling them efficiently, it also calls for acknowledging and going for the options already in place to manage its symptoms effectively.

29th June was observed across the globe as World Scoliosis Day to increase the level of awareness and highlight the importance of treatment of the disease.
Affecting 2-3 percent of the entire US population, scoliosis, or a spinal curve, is one of those medical conditions whose etiology is yet to be understood completely.
Often, due to lack of awareness and information surrounding scoliosis, people tend to ignore the symptom and sometimes even the treatment options.
According to the National Institute of Arthritis and Musculoskeletal and Skin Diseases, of every 1,000 children, 3 to 5 develop spinal curves that are considered to be medical condition (i.e. scoliosis) which require treatment. If not treated properly, scoliosis could lead to other complications linked with unnaturally curved spines.
“Scoliosis often goes unnoticed and untreated, if at all patient report the disease, they do at quite advanced stage, and this is all because of ignorance around the disease. It is unfortunate that despite having advanced treatment modalities, including surgical options effective enough to properly treat the disorder, people hardly recognize scoliosis as a treatable disorder. Most of the time, it is assumed as a part of physiology or is linked with postural issues. This not just worsens the disease itself but also affects internal organs, if not treated timely,” says Dr. Bipin Walia, Director Neurosurgery and Head Spine Surgery at Max Super speciality Hospital, Saket, New Delhi.
Experts explain scoliosis as a disorder that causes an abnormal curve of the spine, or backbone. While a typical spine is straight when viewed from the back, in case of scoliosis the spine can curve in one of three ways -a single curve to the left (shaped like the letter C), called levoscoliosis, a single curve to the right (shaped like a backward letter C), called dextroscoliosis, and two curves (shaped like the letter S).
As far as treatment of scoliosis is concerned, patients nearing skeletal maturity with comparatively lesser degree of abnormal curvature usually do not require any treatment. However, in case of younger patients with significant curve who are yet to grow, it is quite likely that they continue to develop further curve and therefore require treatment. In such cases, where scoliosis is left untreated, it can progress to a stage where it limits the space in the ribcage. This space is vital for the heart and lung to function normally. A diminished space for lungs and heart might lead to respiratory problems and cardiovascular problems.
Exercise or physiotherapy has proved to be of little help in scoliosis management, even if it the case of mild curve. Experts point out that though no particular exercise has been found to reduce or prevent scoliosis, it is still very important for scoliosis patients to keep their back muscles strong and flexible. For mild scoliosis, treatment options primarily include back braces, while for higher degree of curvatures, surgery is the best solution.
Explains Dr. Walia, “In surgical options, spinal fusion is specifically recommended for severe curves that are greater than 45 degrees and for curves that have not responded to non-surgical intervention like braces. Spinal fusion involves placing graft material between the affected bones in the spine to fuse, or join them together.”
During this type of surgery, the surgeon attaches a metal rod to each side of the patient's spine by using hooks or screws attached to the vertebral bodies. Then the surgeon fuses the spine with a piece of bone - a bone graft.
The bone grows in between the vertebrae and holds them together and straight. This process is called spinal fusion. The metal rods attached to the spine ensure that the backbone remains straight while the spinal fusion takes place.
“With recent advances in technology, patients are sometimes released from the hospital within a week of the surgery and do not require post-operative bracing. Many patients are able to return to school or work in 2 to 4 weeks after the surgery. Many are able to resume all pre-operative activities within 4 to 6 months,” says Dr. Bipin Walia.
World Scoliosis Day is an excellent opportunity to spread awareness about the disease as well as to focus attention on advanced treatment modalities which possess the potential of rooting out the problem effectively.
Dearth of skilled and trained personnel is one the biggest challenges the healthcare delivery system of India faces today, according to the Third Annual Wellness Conference report 2011 brought out by the Federation of Indian Chambers of Commerce and Industry (FICCI). The debate is likewise centered on the improper utilization of existing resources which is making matters worse.
According to data published in the Indian Journal of Medical Research in April 2013, seventy per cent of the posts of specialists like surgeons, physicians, paediatricians and gynecologists at Community Health Centers (CHCs) across the country are currently standing vacant1.
“As far as the talent crunch in healthcare goes there are two major issues leading hospital chains are facing today - inadequacy and underutilization. Not only are we faced with a massive shortfall of trained medical professionals, we are also having a tough time putting the available talent to use. Optimum utilization of available resources is a big issue”, says Dr. Vishnu Panigrahi, Group Head, Medical Strategy and Operations, Fortis Healthcare Ltd, New Delhi.
Experts believe that improper utilization of resources needs to be looked at in the context of the huge shortfall of medical professionals in India. According to WHO there are not even seven physicians per 10,000 patients in India. As of now, there is a net shortfall of 600,000 doctors in the country.
“The current underutilization of skilled people in the healthcare industry is undoubtedly a concern about the evolution of healthcare delivery in the country. The sad reality is that healthcare professionals are many times placed inappropriately, not in accordance to their fit for the role. This needs to be addressed immediately”, says Arvind Pandit, CEO VIA Health Resources, a specialist healthcare focused executive search firm.
Although to combat the projected shortage, India plans to establish 200 more medical colleges, experts feel there is a lot more that needs to be done as the problem cannot be sorted out only at the primary health level. Moreover, the perception that there is no problem in the urban areas, and it is only the rural areas that need work is not entirely correct.
Dr. J. Kumar, a senior Neurosurgeon, and Practice Head, VIA Health Resources says, “It’s not just the rural areas, urban areas too face a critical shortage of doctors. Of course, the infrastructure is mostly in place in the urban areas but there are just not enough trained professionals to put it to proper use. The absence of a mechanism to put right people in the right roles complicates the situation further”.
So what’s the solution? Experts agree that in the long term the processes that have already been set in motion by the government will go a long way in helping deal with the problem. However, they point out that in order to provide infallible solutions it is also important to identify channels which could ensure that the proficiencies of existing healthcare professionals are utilized properly. In that context the development of a proper recruitment channel could bring about enormous benefits.
According to Swadeep Srivastava, CEO Via Media Health, “It’s not just about the talent crunch and the problems hospitals face in functioning smoothly. There is also the important question of hiring the right professionals so that healthcare delivery mechanisms function smoothly. Executive search channels which provide sustainable solutions to suit the needs of both the hospitals and the professionals coupled with optimum utilization of resources are the need of the hour.”

Yesterday I wasn't carrying enough money to go to vote because I thought that I would walk but then I saw autos at the stand and so thought of taking one. Moreover, I requested the driver to wait and if it wouldn't be a problem. The driver responded that I can pay later and that there's no issue in waiting and that all the auto drivers at the stand think nice of me. I then had again thought of doing more for the drivers which I would! (y)
I feel sorry for the auto drivers because I've told some of them that they should wear anti-pollution masks on which they've said that they think that the police wouldn't let them due to security issues. How do you feel as an Indian when you read it and think that how come people have led India to like this? Is this also why the aware and wealthy people seem to be moving their children abroad and also having their grandchildren abroad? How would India be like in the next decades when the people have become very weak considering human potential since this would weaken the people? What are you doing to make things better?

5th September 2012: I felt like an Indian street dog considering the world when going through a market which sells including expensive things for common man, however, the area has much filth. I also met a person who goes to the same gym and that she was also there shortly before. I shared with her about my anti pollution mask which I was wearing and that I was trying to work to get the auto rickshaws fixed with water coolers to help drivers not get sick from potentially harmful water that they might get from outside and that to also help them save money and that I was learning from drivers that many support my idea if I can get something affordable and that it can be fixed well in the auto.
Then she asked why I also don't do something for manual rickshaw drivers on which I told her that I'm first targeting one segment and that after that I'll work on manual rickshaw drivers to get them affordable footwear and to see if anything can be done to the rickshaws to make them better like how indoor bikes come in several qualities and that some things in them are critical for us to avoid short-term to long-term injury. Moreover, she and I had also done the spinning class today during which she had said to our trainer that I'm concerned for everyone when I had asked our trainer on her child! Also, a manual rickshaw driver nearby was attentively listening to me as she pointed to me which was nice and that I wish that I can also do much for them.
People keep blaming others including the government and the administration. I don't want to sound like an Indian street dog barking like when I keep clearing my throat due to pollution. I'm doing what I can and that not blaming others or asking for change in government or administration but trying my best to bring change in our society and environment by directly contributing to them. I think that we can contribute much even by making small changes at large scale. 17th March 2014: Why is holika burnt? Perhaps you've read or heard the story. If not then search online and you should easily find it.
Also, what did you do today about "Toxic colours are freely being sold everywhere. These colours contain harmful substances such as zinc, chromium, mercury, lead, mercury, asbestos etc." Are you sure that you didn't have harmful chemicals in colors that you put on others or others put on you? Also, what about the colors in our food that we're eating daily?
Also, what did you do today about "Researchers are beginning to suspect that India's unusual mix of polluted air, poor sanitation and contaminated water may make the country among the most dangerous in the world for lungs." Are you sure that the severely harmful chemicals in your air today are much within the maximum limits including when burning holikas and the chemicals in colors mixing in air, water and land and then going inside us over the long term?
Also, what did you do today about "The NGO Consumer Voice, one of the petitioners in the court, said the quantum of pesticides in fruits and vegetables in India, especially those sold in Delhi markets, was as much as 750 times the European standards." Are you sure that the sweets and other food which you're eating today and over the long term don't have harmful chemicals which aren't even allowed by others in our world for import, however, they keep getting sold here?

It is important to know that it is very difficult to spot the warning signs of an eating disorder. In many cases, the person undergoing it doesn’t even realise that they have an eating disorder. In such situations, one must compare behavioural warning signs along with physical and psychological signs to determine the presence of an eating disorder. Although, all these signs may not be the same for every person, watch out for the following signs.

Extreme dieting: skipping meals, fasting, replacing food with fluids etc
