Polycystic Ovarian Syndrome, also known as PCOS or PCOD (polycystic ovarian disease), is a very common hormonal disorder in women.

Ayurveda classifies PCOS as a Kapha disorder.Vata is responsible for movement of the follicle during the ovarian cycle, the rupture of the ovary wall releasing the matured ovum, the movement of the fimbriae – the finger like projections that guide the ovum into the fallopian tubes and the movement of the ovum towards the uterus.The action of the hormones expresses the nature of pitta, the energy responsible for transformation.Kapha’s heavy cool qualities nourish the development of the tissues that form and support the reproductive system including the nurturing energy supporting growth of the follicle during the ovarian cycle.PCOS is due to kapha blocking vata and pitta, hence movement is obstructed and the transformation process is suppressed.

Artava dhatu affected by the heavy sticky qualities of kapha and ama creates srota dushti in artavavaha srota. Apana vayu in artavavaha srota becomes stagnant – sanga, due to excessive kapha and ama accumulation blocking the channel impeding the flow of vata in the ovarian cycle. Because vata is blocked, pitta is blocked as well as pitta in order to act as the intelligence behind transformation needs the movement of vata in order for its energy to have potential. As pitta is blocked the hormones that carry the energy of transformation are unable to initiate their action. The accumulated kapha is expressed in the formation of the cyst in the ovary as it takes on a heavy white sticky quality expressing kapha and ama.

Due to vata and pitta being blocked in artava dhatu the other functions of both these doshas begin to become aggravated. Pitta aggravation at the level of bhrajaka pitta and ranjaka pitta manifests as acne and increased body hair. Menstrual problems manifest due to the aggravation of all three doshas but namely apana vayu.

# Treatment of PCOS With Allopathic Medicines :

Allopathy treats the condition on lines of hormonal imbalance and the major treatment includes HRT (hormone replacement therapy) where subsequent hormones are administered after proper evaluation of the patient. Apart from that, diet and certain exercises are also recommended. Apart from that, there is a growing belief that PCOS occurs due to malfunction of the hormone insulin and hence diabetic drugs like metformin may also be prescribed by your doctor for the same. Surgery is also an option exercised where the ovary is punctured or the cystic tissue above the ovary is destroyed for better hormonal gain. However, all these have their own after effects and you must evaluate all options about it by talking with your gynecologist and personally researching on the net. This rule not only applies to allopathy but also to other streams of medicine as after all it is your body.

In taking hormones, it further depletes your endocrinal system and puts them in sleep. The stuff that has to be naturally released by the body, once delivered with artificial means puts the body in a lazy mode. The HRT treatment may give you good results but is not recommended in long run as it can cause other problems.

# Now taking in account the above-mentioned treatment modalities, we can place a pattern for proper evaluation and treatment modalities for PCOS.

Eat at regular intervals. For example, if you have breakfast, lunch, and dinner between 8 to 9, 12 to 2 and 8 to 9 respectively, see to it that you follow these timings as the thyroid gland become trained to secrete juices in these times. Any variations and it puts pressure on the glands. Doesn't matter even if you gulp down a glass of milk, but the timings must be maintained.

Salt restriction is another major aspect. Prepare food without salt. Take as much as you want in the plate (only one helping) then add salt to taste. Prefer saindhav salt (Himalayan crystal salt).

Have a papaya for breakfast/dinner at least 2-3 times a week until your cycles regularize. Apart from that also intake porridge prepared from Ragi extract (nachini, Eleusine coracana). prefer crystal sugar (Khadisakhar, Mishri) for taste instead of normal sugar. Weight watchers can prefer sugar substitutes however crystal sugar is very safe.

Avoid sleeping after lunch. Have a glass of buttermilk (prepared without salt).

Stay away from direct/indirect use of the following items; Brinjals, Peas, Peanuts, cauliflower, seafood, pickles, curds (buttermilk is okay), green chilies, mustard, bakery products (pastries okay).

# Here is List of some simple herbal decoctions, which if taken over a period of 2 to 3 months will definitely help:

Take Ashwagandha roots (Withania somnifera; entire about 150 grams), Arjun bark (Terminalia arjuna; about 70 grams) from a local herb shop. Divide both of these into 30 equal parts. Now boil 1 part every morning in 3 cups of water (approximately 150 ml) and reduce to 1 cup. Now filter this mixture and add 1 cup of cow's milk to it. Boil the mixture again over a low flame until milk remains. Add 2 cardamoms while boiling. Consume this milk early morning empty stomach. Little sugar can be added for taste. The remaining filtrate of Ashwagandha and Arjun can be used similarly in evening. During summer months, Shatavari (Asparagus racemosus var. javanica) can be substituted for Ashwagandha.

After 30 days or 35 days if the periods do not commence have the following mixture for 3 days, 10 grams Nagkeshar (Mesua ferra flower stamens), 10 gms crystal sugar (Khadisakhar, Mishri) and 10 gms Cow's ghee (clarified butter from cow), it must be mixed and consumed with milk once a day. The above combination is for one day. If the periods do not commence even after this medicine, then do not worry. Keep taking the above medicine. Eventually they will come and with proper ovulation.

Castor Oil Pack : A Castor Oil Pack is a cloth soaked in castor oil which is placed on the skin to enhance circulation and promote healing of the tissues and organs underneath the skin.When it comes to fertility Castor Oil packs could be a great, supportive and relaxing therapy .

The above simple remedy coupled with diet will definitely help and let you lead a normal menstrual life with kids and no infertility. In case you feel you need to evaluate this option more, try and consult a good Ayurveda physician. Having customized and specialized treatments will help you gain more. For example:

Generic preparations like Ashwagandharishta, Ashokarishta, Kumaryasava, Chandraprabha Vati, Pushyanug churna, Phal Ghrita, Raupya Bhasma, Vang Bhasma, Abrakh Bhasma, etc. are useful.

Panchakarma techniques like Basti, Uttar Basti, Vaman, Virechana, Nasya are also undertaken.

Certain yogic exercises like Simhasan, Vajrasan, Shalabhasan, Sarvangasan are also helpful and may be coupled with the above medicines.

Besides the above remedies, the simple Ayurveda method to check if the periods are adulatory and with proper hormonal functions is to take a few drops of menstrual blood on a clean white cloth and wait for few seconds. Now wash the cloth with plain warm water (no soap, detergents) and observe if it leaves a pickle like stain. Clean periods as per Ayurveda must not leave stain on the cloth. Once you achieve that, you can then try conception !

For many who prefer to maintain their weight and their health without the use of supplements, they need to consider that the Industrial Revolution has resulted in a sedentary lifestyle (decreased physical labor requirement) and refined/processed foods which has resulted in a decline in weight management. Our environment has changed faster than we can adapt, many of us as we age store excess energy (fat) as if to prepare for a case in which we couldn’t find food regularly or needed to store energy for physical activity as would have been the case over a hundred years ago.

However, the fact is that we spend most of our time on our butt thanks to technology and it is harming our health. As much as some would like to complain about the health care costs of lifters injuring themselves and requiring surgery, the fact is that the majority of health care costs are the result of heart disease and cancer (the top 2) and many of the major complications of obesity are right below those top two on the list (heart disease is the #1 complication associated with obesity). So if technology has caused us to become sedentary and it has negatively impacted our health, why not fight back with technology?

I’m not talking about becoming dependent on pharmaceutical drugs to maintain your health. I’m talking about the supplement industries ability to aid in motivation, exercise performance, recovery and the regulation of calories. Everyone has either heard it before or asked the question “can I combine these two pre-workouts?” or “Can I take this pre workout with this fat burner?” Just to make things clear...no stimulant product in the recent history of the supplement industry was designed to be combined with another stimulant supplement.

Part of the reason for so many misconceptions is the result of advertising; unfortunately, to grab the attention of potential customers describing how products make you feel in the most colorful ways possible is part of the game. However, when advertisements start guaranteeing muscle gains in a certain period of time or promote a product as being so strong it could possibly be harmful to your health; that’s when things go too far.

Now for those who don’t believe supplementation is effective at all, it should be known that many of the ingredients and analogues of amino acids are based on things found in food in concentrated amounts for performance. Outside of creatine and protein (especially the BCAAs and the other amino acids that are in protein) there are very few supplements that have a direct correlation with muscle growth.

The majority of supplements, if you look closely, are associated with boosting performance or recovery to aid in your efforts to gain muscle. With all the research being done on supplements and steroids the one thing that can get overlooked at times is the fact that muscle growth has a direct relationship to training. Even if you took steroids, if you sat at home doing nothing, there would be no muscle growth to speak of.

For those who don’t believe in supplementation who feel they are of a higher education level, much of the research disproving the use of supplementation is based on popular supplements like L-arginine being disproven for many of its major uses pre-exercise in terms of increasing the pump or increasing performance (2). But for every bit of researching disproving an ingredients application, there is research supporting ingredients like Citrulline Malate and GPLC (Glycine Proprionyl L-Carnitine) for the same use with benefits for muscular endurance and improved performance.

Also, some of the research discrediting supplementation is ill conceived. All too often research will be done on an ingredient to test for increases in lean body mass after only a short period of time (4-8 weeks) on ingredients that were only meant to promote increases in endurance/performance or recovery. And they use this as evidence to disprove supplementation use when, as stated earlier; outside of creatine and amino acids much of the ingredients in supplementation promote increases in performance or recovery which allow the user to make gains in muscle growth if diet and training are in line with their goals.

A particular case in which this is evident is in a research study done to determine the effectiveness of a protein/carbohydrate supplement, the researchers attempted to discover some sort of correlation between the supplement and performance despite the fact that it is primarily promoted for recovery. They did however discover markers for increased recovery thanks to protein/carbohydrate supplementation (3).

Another aspect of the supplement industry that is criticized is the pre-workout. It is well known that caffeine interferes with creatine’s effectiveness if they are taken at the same time; however this is not enough to discredit the use of stimulant based pre-workouts. Your strength is largely central nervous system based, so caffeine’s ability to decrease pain associated with exercise and result in an acute increase in strength is very real (4).

Also, in a study done on sedentary (does not exercise on a regular basis) men, it was determined that a low-calorie energy drink aided in their physiological adaptations to exercise (5). This is very important when you consider that it improved their body’s ability to adapt to a stress (exercise) that it was not previously trained to be able to handle.

Supplementation even for beginners can be important for helping them to cope with the physiological stress associated with returning to exercise. As far as the concern for taking too many supplements, although too much of something is a bad thing (see ‘The Key Principle for Life, Dieting and Training: Moderation’) much of it is unwarranted. The largest known concerns based on research for supplementation are that fat soluble vitamins (like A, D, E, and K) have known toxicity levels and that taking too much of single amino acids ex: L-Leucine, can interfere with the absorption of other amino acids (6).

Another concern that the NCAA took control of was excess protein supplementation which lead to dehydration. Now the NCAA’s new ruling is to not recommend servings of protein supplements in excess of 40g per serving. However, past this, as long as supplements are taken as recommended there isn’t too much cause for concern. creatine is constantly proven to be safe and effective and recently popular amino acids in research such as L-Carnitine L-Tartrate have also passed their preliminary tests for safety (7).

So in the end, while it is true that you’ll have to do a little research on a supplements ingredients before purchasing and falling into the hype; that doesn’t mean that supplements are ineffective. For every study that discredits a supplement, there is one that proves another ingredient worthy to add to your training regimen, and for every proven ingredient that is discredited for a particular use; if applied appropriately it can be irreplaceable in your supplement stack.

The key thing to also remember is that very few products have a direct relationship to muscle growth; they are mostly training aids, so there should be no fear of becoming too bulky or looking like the bodybuilders in the ads because you are using a product to increase your endurance and another to increase recovery.

Feedback:
It is a good practise to know what your members want and the problems they are facing. You won’t always have 100% satisfaction, but getting closer is not bad. Close down the problems that the clients face.
Give ears to their opinions, not all the clients will always come to the desk and speak up, but those who do, understand and help them out.
Importantly, one bad review from the client, is not the end of it. You can always improvise and come back better.
